field trip permission
FORM

Dear Parents,

ILM Academy has a field trip planned for Blue and Gray Room students to visit the Volkening Heritage Farm on Tuesday, October 20, 2026.

We will use a chartered school bus to transport our students and staff to the Volkening Heritage Farm.

Please fill out a separate form for each child.

Do you give permission for your child to go on the trip in a chartered school bus?
