PHOTOGRAPH CONSENT
FORM
From time to time we will take photographs of the children in the school during
activities or at a special event such as field trips, class performances, parties, etc.
The purpose is for having it in the classroom or on our school album both in soft
and hard format and a few soft copies to be posted on our website.
We also have appointed a professional photographer who will come in during
the year to take a class picture and individual picture of your child. Please
provide your consent on this form below.
I, the parent of the below-mentioned Student give/do not give
permission for my child’s picture to be taken at ILM Montessori School.
We at ILM Montessori regard your Child’s safety and privacy as paramount.
